Care Navigation and Admin Team
Our Patient Care Coordinators (PCCs) are a very important part of the practice, they are the first point of contact for general enquiries. When a patient contacts the practice for a health concern, the PCC team will ensure an appointment is made with the most appropriate member of the team.
They can also provide basic information on recent test results, direct you to the correct clinician, help to answer general questions you may have including prescription requests and they support the incoming and outgoing letters and referrals.
The team constantly strives to keep all the information in the surgery up to date and confidential.
