Information for children aged under 6

Just One Norfolk are able to help with any questions or concerns about your child’s health, wellbeing or development. Their clinical team are able to support you over the telephone, video call or text, and work with you to understand how best to support you and your family. They have access to health visitors, Speech and Language Therapists, and a range of other specialists. They are able to refer to other services such as hearing tests.

For us to refer your child to the Community Paediatricians, we would need the following information:

  • Ages and Stages Questionnaire, or Schedule of Growing Skills. The team at Just One Norfolk can help you fill this out.
  • (optional) Early Years Foundation Stages tracker, if you have it. This is normally filled out by nurseries, pre-schools or childminders.
  • What you have noticed at home. If you are concerned about ADHD, look at the list of symptoms at NHS: ADHD symptoms, and tell us what symptoms you have noticed. If you are concerned about autism, look at NHS: Signs of autism in children.
  • (optional) Any other reports that you have from other professionals, such as reports from the school, or speech and language therapists or child psychologists.

Once we have received this information, we can arrange an appointment with a GP to discuss making a referral.
