Temporary patient registration

Use this service to temporarily register with the surgery.

You can use this service if you:

• want to register at the surgery
• live within our catchment area

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your NHS number
• information about your health
• other personal details and demographics

This information will be kept on our system and we will only register you when you need (urgent) medical assistance. If and when this is the case, please call the surgery and we will temporarily register you.
